Differential features of metabolic abnormalities between medial and lateral temporal lobe epilepsy: quantitative analysis of (18)F-FDG PET using SPM.

Differentiation of medial from lateral Temporal Lobe Epilepsy (TLE) is important to allow for targeted treatment. This study used PET scans to identify hypometabolic regions in the brains of epileptic patients. Results showed that the hypometabolism in the temporal lobes was detected ipsilateral to the seizure focus in 76% of the TLE patients but on the contralateral temporal lobes in 32% of the patients. The authors concluded that the specialist PET scanning technique used in the study could accurately locate the seizure focus which helped in the discrimination of medial TLE from lateral TLE. They suggest that lateral TLE should be considered when glucose metabolism is relatively preserved in the medial temporal structures.
